Documentary Requirements?
•Applicant's valid ID together with the
enrollment or assessment form;
Certificate of Registration or Enrollment; •Valid School ID
•If college level: the Statement of Account (SOA)
•The applicant needs to obtain a
Certificate of Indigency or Residency
from the barangay that the applicant belongs to.
•Original and photocopy of the valid ID of the applying student and the parent;
•Photocopy:
•Enrollment Form
•Registration Form
•Compile all of the documents in an A4 folder;
•Submit it to your nearest DSWD Office.
