DSWD, Requirements for 2nd Batch Education Assistance

ATTENTION Batch 2

EDUCATIONAL ASSISTANCE 

to Students Under 3rd District

List of Requirements 

for Educational Assistance Payout on March 2, 2022 @ Mantalongon Covered Court from 10am to 3pm

For students who have School IDs particularly the continuing or ongoing students:

1 Certificate of Enrollment (AY 2021-2022) individually and updated date

2 Certificate of Indigency (purpose: for whatever legal purpose it may serve) updated date

3 School ID

For students who have NO IDs particularly 1st year and transferees:

1 Certificate of Enrollment (AY 2021-2022) individually and updated date

2 Certificate of No ID from the school individually and updated date

3 Certificate of No Valid ID from the barangay updated date

4 Certificate of Indigency (purpose: for whatever legal purpose it may serve) updated date

Note:

1 Please prepare Original and two photocopies of all required documents

2 Kindly affix 3 specimens of your signature under the photocopy of your School ID

3 Bring your own blue ballpen, face mask, face shield, alcohol, & hand sanitizer

4 Failure to comply with all the above-mentioned requirements means FORFEITURE

For those requirements mentioned individually and updated date please comply as soon as possible since it will be collected during the payout.

List are to be followed posted since the office are still waiting the final list from the DSWD.
